How do you puncture a body?

This obviously something to avoid, but how does it happen?

overtightening the front screw that holds the grip frame onto the body.. usually when the screw is without a washer.. the tip of the screw eats right through the soft aluminum into the bottom tube

Got it, thanks. I have a dragon body and a 2k5 frame. If that screw is more than snug, I cannot remove or attach my clamshell cover, so hopefully I will not see that ailment.
